The Social Services department offers general social work services to all inpatients, out-patients and their families at Millcreek Community Hospital, including:

• Discharge planning
• Placement services
• Psychosocial assessments
• Short term counseling
• Information and/or referral
• Transportation needs

In collaboration with appropriate clinical and administrative representatives, the Social Services Department seeks to ensure that each patient receives a continuation of health care needs after hospitalization and is matched with the appropriate resources in the community.

Unlike most hospitals, where nurses are charged with providing pharmacological recommendations to patients upon discharge, MCH's pharmacists and pharmacy residents provide discharge pharmacy counseling to patients being discharged.

What you need to know

• The social worker and pharmacist will come to you on the day of discharge.

• The Social Service Department is staffed Monday through Friday from 8 am to 4 pm.
